Can I get a copy of the consent form I signed?

Yes. You may print a copy of the informed consent form for participation in the Brain Health Registry before or after agreeing.

Once you’ve agreed, you may view and print a copy of the consent by logging into your Brain Health Registry account and going to your profile page, by clicking Your Profile in the upper right-hand corner. Under the Consents section on the left side menu, you have the option to view and print a copy of the consent form.
